You understand and agree to respect the private nature of these sessions as well agreeing to not publicly claim or share practices that are created for your personal development and unfoldment by Jackie Duffey at The Holistic Swan LLC, as your own via the world wide web or social media. All sessions are for adults 18 years old or older. CANCELLATION POLICY: Please note 48 hours notice is required if you need to reschedule your appointment or a 50% session fee will be charged. No shows will be charged full session fee. Jackie Duffey at The Holistic Swan LLC reserves the right to cancel a session should an unavoidable reason occur and will do her best to reschedule an alternative date. This form will be taken as an agreement to the terms and conditions outlined above.
